Over the last 3 years Leanne Dowse (University of New South Wales), Kathy Ellem (University of Queensland) and a team of others* have led the ‘Lost in Transition’ project, an ARC Linkage project which aimed to understand the experience of and needs of young people with complex needs in transition.

Roundtable agenda will include

  •       Practice in transition
  •       Facilitators and barriers to Good Practice
  •       Possibilities of Practice
