 Job Description

Inclusive Health Partnerships provides and promotes healthcare integrated with housing and homelessness services across a range of partnerships and referrals. Working to prevent exits into homelessness and frequent presentations at hospital emergency departments, with increased access to healthcare and wellbeing. Inclusive Health Partnerships comprises of two components including the Inclusive Health and Wellness Hub – a general practice and wellness clinic (partnership between Micah Projects and Tzu Chi Foundation) and the Outreach Integrated Care Team.

We are seeking a compassionate and dedicated Registered Nurse to join our Inclusive Health Partnerships Cluster. This role involves providing high-quality nursing care at The Park Hotel in Spring Hill, a crisis accommodation site managed by St Vincent de Paul. The position includes assessing residents’ physical and mental health needs, providing immediate medical care, health education, harm reduction strategies, medication management, and advocacy.

This is a fixed term full time maternity relief employment opportunity with conditions and salary at Level 5 of the QLD Nurses and Midwives EB11 Agreement.

 Desired Skills and Experience

Key Responsibilities:

  • Conduct health assessments and support residents in accessing healthcare services.
  • Provide immediate medical care and respond to emergencies.
  • Offer health education and preventative care information.
  • Implement harm reduction strategies.
  • Manage and support medication adherence.
  • Collaborate with St Vincent de Paul staff and refer residents to community health services.
  • Maintain accurate medical records and documentation.
  • Support residents in linking with the weekly GP clinics provided by Inclusive Health and Wellness Hub.

Essential Criteria:

  • APHRA Registration.
  • Valid Driver’s License.
  • Minimum of two years of clinical experience.
  • Effective communication and clinical problem-solving skills.
  • Commitment to child and vulnerable adult safeguarding principles.
  • Ability to work collaboratively within a team and with external stakeholders.

Desirable Criteria:

  • Postgraduate qualification in a relevant specialty.
  • Experience with chronic disease management, addiction, and/or mental illness.
  • Interest in community/primary health nursing.
